Thanks for making this request. In order to understand the use-case more better I have a few questions:
1. Whats the role of the different people working collaboratively on the same file. Are they always only designers or are other personas also involved.
2. What is the use-case here, some parts of this request suggest an Editorial collaboration whereas others suggest collaboration amongst different designers. I am trying to separate these 2 requests out.
3. What are the different kind of files (brochures, books, magazines etc) for which this is required.
4. Does it need to be a web based service, or having the user be able to collaborate on one shared file using InDesign or InCopy on the Desktop will work? What is the advantage you feel over a web-service over the desktop app?
5. How do you workaround this need today? What are the most important problems with…Colin Flashman

According to the ISO-32000, the Hyperlinks are NOT supposed to be populated into the Alt-Text field. They should be entered into the Contents Key (see attachment) instead.
InDesign currently does place them in the correct place [see screenshot attached].
While someone using Acrobat can also add them to the Alt-text field, this is technically less correct than entering into the Contents key but either will pass an Accessibility checker.InDesign implemented this correctly based on the above information (provided by Accessibility experts, like Dax Castro, thanks Dax).

Fix for this issue is now available in the latest update of InDesign 2021 – that is, InDesign 16.2.1
In the Creative Cloud Desktop application, click on the Updates section on the left. You should be able to see the Update available for InDesign. Please update your InDesign application to this version to try out the fix.

I'd include the ability for preflight to also FIX the issues at hand. e.g. it detects registration being used in type... how about a "convert to black" button when that is detected? Another example - it detects images not bleeding off the page, though there is enough image to do so - how about a button that can be clicked that says "add bleed" and it fixes the issue? Reporting on the issues is one part, but fixing the issues would also be great.

Can you help us understand the key problems that you face in the absence of ability to send an anchored image backwards?
What’s the current workaround that you have to follow?
What are the use cases wherein you need that the text appears specifically on top of inline anchored image?
This information will help us evaluate the functionality better.

At the moment it is impossible and there's no real workaround that isn't inconvenient or easily understood by everyone who may have to work on the same file.
The behaviour of the anchored objects has two behaviours that are inherently frustrating:
1) The anchored object will appear one stack above the type it is being anchored to; and
2) Will not text wrap around any objects that appear on the same line or earlier in a story (Pariah Burke's video does a good job explaining this: https://youtu.be/cwgdGM4dQ6A and provides a workaround that can easily be undone by anyone else unfamiliar with the workaround who has to work on the same file)An instance of why you may want an anchored object to appear underneath the text could be a stylised heading like the attached photo. Currently, this is done by having the graphic on a stack below the type that is simply floating; but if the type moves, the graphic will stay where it is. The purpose of anchoring the graphic is that if the type moves, the picture follows it; but currently this is only in the X and Y axis, not the Z-axis of stacking where the only level of control is for the anchored object to be one stack above the type.
