   Same issue, except that the shift key doesn't help, it still only moves the image.

   This issue is fixed in the latest InDesign Release 21.1

    We can't mark paragraph styles as captions for accessible PDFs (which we should be able to by the way). But the only way to correctly export captions is deeply flawed. There are laws surrounding accessible documents, so get moving!
