Thanks for the detailed feedback on this thread. Reopening it, because we're still actively working on the issue. A few updates:
Relinking: the case where manually entered alt text was overwritten by generated alt text on relink was fixed in 21.4. If you're still seeing manual alt text get overwritten after relinking, reply here with a sample file and your InDesign version and we'll investigate directly.
Remove all generated alt text: an option to remove all generated alt text from a document in a single action is on the way.
We hear the wider feedback here too, and we're keeping this open while we work through it.

Nearly six years later, you can see that Adobe isn’t considering responding to this. There are many InDesign development lags and other indications that indicate the company is not putting adequate resources into InDesign. I feel sorry for their dev team. There are clearly products that Adobe is motivated to develop and sell, but their commitment to print has been fading for years, and InDesign is the canary in the coal mine.

Bottom line, I want “Marks and Bleeds” options in Share for Review. This addition would make the feature 90% more feasible for my workflow and move me and my clients out of Acrobat.

This is a rage-inducing UI mistake. I'm an InDesign professional. I am being driven insane by these obstructive interruptions in workflow with NO WAY TO DISABLE. This problem is being ignored and proving the uselessness of both the community forums and UserVoice.
Listen to your users. Disable this nanny "feature" and let us get back to work!!!
