I've investigated the issue and identified the lowest common denominator - it's reproducible on different machines in both InDesign 21.2 x64 and InDesign 21.3 x64 Beta.
The error must lie in a certain quantity of elements/pixels/rendered areas — I’ve created a short 3:45-minute video that clearly demonstrates and analyzes the symptoms and variations (to be found following the link above).

The issue of “Unmapped Comments” has been fixed.
Please use Share for Review feature and check when you make comments on the web if they are shown on the correct Spread.

The fix for this issue is available in the latest InDesign release 16.2.1
Please update to this latest version to get the fix.—

As of today, by using the InDesign 21.4 x64 Beta (the most recent pre-release-version should be fixed too), all of my complex files update flawlessly. Even with the test file I posted earlier, the sharing link updates flawlessly and as often as needed (even when I inflate the file to 200 pages and 500 linked complex PDF files).
That’s why I’d recommend to give the new beta a try (note that the main version still contains the bug; only the beta is working right now).
A quick note about the beta tab: if the tab doesn't appear, try typing "beta" into the search bar at the top and clicking on the corresponding result. If it still doesn't appear and you're a member of an Adobe Team instance, the beta apps may be disabled in the deployment settings of your CC client.
