I can also confirm this serious bug.
I have been trying to recreate it. But it just occasionally pops up. So I can’t provide any accurate parameters as to when it occurs.
So far it hasn’t happened in documents with less than 10 pages and it has only happened in documents where I imported Word documents. Thing is: Most of my InDesign files that are more than 10 pages have Word documents imported. So that’s of little help.
Has this bug occurred for anyone here in a document with fewer pages or in a document without any imported Word files?

This continues to be a problem in 18.0. Again, for me it’s files upwards of 10 pages with imported Word files. I have not been able to narrow it down any further.
Please fix this bug, it’s making work on long documents very difficult and error prone.
