Alternatively, a setting to "Include bleed from facing page" would help, which could add bleed lines to the gutter area. This would prevent the situation where designers would have to duplicate items that cross the gutter, as they would if the pages were separated.

Yes, please! I was trying to do this just yesterday. In my case, I was trying to fix the issue where, in the case that a whole paragraph is underlined, InDesign adds an underline to the white space at the end of each line. It looks messy and ragged. This would be easy to fix with a GREP style (character style to remove underlines, applied to a white space at the end of each line). Except there is currently no way to match that.
