    I can confirm the bug with InDesign CS6, CC2015 and CC2021 on both Windows and Mac.
    I've made a sample file myself, exactly the same issue here. Index entries are ignored until the last anchored text frame appeared.

    So far, I've tried:

    - converting docx to doc
    - converting docx to rtf
    - importing with CS6, CC2016, CC2017 (tried both Windows + Mac versions)
    - cleaning the docx with words internal "clean document" function

    Please provide a fix for this.
